Six miscarriages

by Kristina
(Des Moines, IA)

I have 2 girls, but have had 6 miscarriages. I had my first 3 miscarriages before my daughter was born in 2002 and had my second daughter in 2004. With both I was on progesterone suppositories. I have had 3 other miscarriages since then and have been on the same treatment.

The last one I started 2 days before my missed period. We found out we were expecting in the ER. The pregnancy before my first daughter I had to do progesterone injections. After that miscarriage we got pregnant within 2 weeks.

Please explain this to me. How I can have 2 healthy full-term babies, but every other we have lost? We have had every study known to verify that nothing is wrong with our chromosomes. Why did the treatment work with our girls, but has failed every other time since our youngest?

Hi Kristina There are a few possible explanations. You are four years older, which is not much, but maybe the dose of progesterone wasn't high enough, you don't mention how much you were using. As you are at high risk, the dose should be no less than 200mg/day, although 400mg/day would be safer.

Stress causes miscarriages, as progesterone levels drop when stressed, were you going through more stress than normal? And for how long were you on the suppositories? The full pregnancy or just the first two months? Although these are the most critical months, it's not until 8 weeks has passed that the placenta starts making progesterone. If at this point you stopped using them it could have been before your placenta was making enough to support the foetus.

I always feel it's safer to use progesterone for the first 3-4 months, rather than the first 2, particularly in someone with recurrent miscarriages. And finally, if you did stop the suppositories at some point, did you taper off the dose gradually over a month, or did you stop cold turkey, as that would instantly cause a miscarriage. The drop in progesterone would have been too great, unless the placenta was making enough to prevent it.

We do have a web page on pregnancy which gives more info, please have a look at it:

Recurrent miscarriages are often a result of too little progesterone. We do have a web page on pregnancy, please see here. And here. Unfortunately women are only put on progesterone after they have fallen pregnant and not before, and I believe this is what you need. Stress causes progesterone levels to drop sharply, so do try to avoid stress. I can't advise you to use progesterone or not before you see the gyne, but once you've read the web page you will be better able to make a decision. I can assure you many women have used progesterone and have successfully delivered a baby. Take care, Wray
