Advice on prescription Bioidentical HRT

I’m so happy to have found this site, thank you!. I’ve been suffering with hot flashes and bad sleeps for 11 years. Conventional hrt which was Bi EST 1 mg cream and progesterone 100 mg capsule did nothing but make me feel anxious at night. I stopped all and decided to live with it. Took nothing for 5 years. Last Oct. met with a hormone specialist nurse practitioner, who put me back on that same dose as a starting point, then after 3 months had a saliva test done. I had estrogen dominance. It seemed the estrogen at the 1 mg was in the range but progesterone was very low. The practitioner doubled both doses of each, so 2 mg of bioest and 200 mg progesterone. I think she should have doubled just progesterone. Didn’t make sense to me. I asked her if a progesterone cream would be better than the capsules and she said no because the cream doesn’t break the brain / blood barrier and it’s a time release capsule. Anyway after 11 days of this doubling of the estrogen and progesterone, I had a terrible panic / anxiety attack and had one the same time the next night. I’m sure it’s too much estrogen.

I am now doing the dosing on my own. Doing 200 mg of progesterone and 1 mg of the Bio est every second day. I can’t understand why she would have doubled the Bio est when after I was taking the 1 mg the levels were within range. I’m in Canada and before hormones of any sort my reading on blood work was Estridiol, 60 ( so not sure if this means my number was 60 or below ) and the range is 60-854. Progesterone was 0.7 and I’m not sure what the range is. Says follicular < 3.7 so not sure how to read this.
After taking the first hrt my estradiol on the saliva test, at 1mg. was 3.3, progesterone 143, Pg/Eg Ratio 43.3 and the reference is 200-600.
I also have low dhea at 57, the reference being 106-300.

I’m interested in any comments or suggestions you may have.
Do you think I really need any estrogen? If my actual number was 60 on the blood work before any hrt, my estradiol is low but in the range.
Do you think the anxiety attacks were because of too high of the bio est she prescribed?
Do you believe capsules are better because of what she said breaking blood brain barrier?

I hope all this and the numbers aren’t too confusing. Please ask me to elaborate on anything.

Hi Pamela

If you read this website you will see that it does not promote the use of estrogen at all. There are well over 100 estrogen mimics in our Environment as it is. It adds to Estrogen Dominance symptoms.

Oral progesterone is one of the least effective methods as 96% gets destroyed by the gut and liver. A good progesterone cream such as Natpro which is a 3% cream is far more effective. See here. It would also explain your Anxiety as not enough progesterone is being used. Progesterone therapy is not an overnight fix, it can take 2-6 months before the body adjust.

Vitamin D3 is very important, a deficiency reduces the benefits of progesterone. Nothing less than 5,000iu's per day is needed. Optimal range is between 70-100ng/ml. Magnesium and vitamin K2 with no soy are important co-factors needed when taking vitamin D3.
